One way is to select the grandchild, right click and link as child to the grandparents family line. Then select the link and change the Pedigree Link to Adopted or Foster child.

Alternatively you can use a Social or Emotional Link. All of these can have comments added which will show as tool tips or you can add a text box with explanation
