How can I set my default "Name Format" in the Display Tab to:
Title, First, Last, (Last 2), Suffix
I thought this format would be available (mostly because it makes sense), but after looking in the drop down list, I could not find it. Any help ???

I annot see that format either, however you could choose: Title First M. Last (last2) Suffix instead and then edit the Display Name field directly to remove the middle name initial (M.) when present.
