A gedcom file is text only so does not contai photos. It does contain the links you need to display the pictures.
You need to download the images separately and place them in a file referred to i the gedcom

The Gedcom standard does not provide a way of specifying which of the pictures is the main, or primary, picture for an individual.

However some packages can make use of non-standard Gedcom tags for this purpose and the {EN} Export to Gedcom report allows you to specify the tag under the Detail Level tab of the Parameter Settings dialogue. You can set it to either _PRIM Y or PRIMARY. Only the first of these is allowed under the Gedcom Specification as a user-defined tag (introduced by the underscore _) but some packages expect a PRIMARY tag.
