I think I should have specified I wanted to see a blank questionaire form for MEDICAL information i.e. to see which topics are addressed I have already done the other i.e. Family, Individual, pedigree data

I did not have any specific medical conditions in mind... I have seen a couple of different lists mainly those which might be used by a medical person collecting a patients medical history... genealogy programs have "blank questionaire" for birth, place, etc. Since GenoPro addresses medical questions, I assumed there must also be a "blank" form with e.g. yes/no, date ID-ed, etc for various "conditions" e.g. heart, allergies, cancer, etc. My intent was to ask relatives (who have already given me vital stats.) if they would complete so I could do MY family medical history (advise, is up to three generations to include cousins, etc.)
