Suggestions on how I show I have contact info for individuals?

I was using each quadrant (highlighted in yellow), as a quick way to show I have someone's:
  • Phone Number,
  • Email,
  • Home Address, and
  • Facebook page or website.
I colored the quadrants in yellow so they were easy to see through, but I know see that I've marked a lot of people as having Sexually Transmitted Diseases (lol).

Is there another way I can quickly show I have the info above without opening every person to see what info I have on them?
