Picture Index

Author	I have places and sources that have pictures, but the Picture Index doesn't seem to be generated correctly for these categories.

Taking Places as an example, I think the problem lies in Lang.vbs with the WriteHtmlTocPlaces subroutine (and the equivalent routines for sources, etc) I seems that a place node will only be added "If Not fPicturesOnly", so this routine won't ever work when called from toc_pictures.htm, since it calls the subroutine with fPicturesOnly set to true.
