You can simply type the title you require for a particular individual into the title field. All such 'custom' titles will appear in the drop down list for subsequent individual and for amendment to existing individuals.

Is it possible to add items to the drop-down list in the person's Title field (Mr, Mrs, Dr, etc.)?
